Instructions for Using Online Forms

Click the link for the form you need to access. All forms allow you to fill in the fields and save them. Some forms will require you to download them to the Desktop version of Microsoft Word before the fields can become fillable. For guidance, you can review the instructions for filling out Microsoft Word Forms.

Forms can also be printed and filled out by hand. In either case, you should complete the form, sign, and return it to the Registrar’s Office. You can bring it to the office in Old Main room 139, email it to [email protected] or mail it to:
